Be prepared to provide as much pertinent information as you can, including the date the fraud occurred and a description of the fraud.
Tell your employer about the fraud, as they are required to file documentation in order to support the claim. Below is a pre-written email to send to the employer notifying them of the claim. The employer will need to be on the lookout for notices about unemployment claims filed by employees, so it's necessary to keep them informed.

Next, you will need to report the fraud to the Department of Labor in the state where the claim was filed. Since the fraud occurred in Nebraska, visit the Nebraska Department of Labor (NDOL)* website and fill out the online form.

If you received a 1099-G IRS form reflecting unemployment benefits but you didn't file for unemployment, you will see that Box 1 on this form will show an unemployment benefit amount. Or if you did file for unemployment benefits and the amount shown in Box 1 of the form exceeds your records for the unemployment benefits, the state will need to issue you a corrected 1099-G tax form and update the tax record with the IRS on your behalf.
You will need to contact the issuing state agency to request a revised Form 1099-G showing you didn't receive these benefits or that the benefit amount is incorrect.

A special email was established for identity theft discovered through 1099 tax forms. Any individual who received a 1099-G from NDOL, but did not file an unemployment claim, should report suspected fraud by emailing NDOL.1099fraud@nebraska.gov and filing a police report.
NDOL does NOT use text messages to issue determinations regarding a claim, nor does it contact claimants via social media regarding their claim. Individuals should only interact with NDOL's official Facebook page, which has a blue checkmark certifying the authenticity of the page. Anyone who comes across an imposter Facebook page or fraudulent message of any kind should report the incident.

We strongly recommend contacting the Social Security Administration directly to set up an account and monitor your yearly earnings. This will help to ensure your Social Security number isn't being used fraudulently.
